Inclusion criteria
Adults aged ≥18 admitted with acute illness who have a diagnosis of diabetes (Type 1, Type 2 or other) and are prescribed at least one diabetes medication on admission.
Exclusion criteria
Patients without diabetes; patients on diet-controlled diabetes with no medications; planned elective admissions for non-acute reasons; patients receiving end-of-life care where review is not appropriate.
Sample size target
60 cases across AMU, ED, SDEC and Care of the Elderly.
Planned re-audit date
3 months after intervention rollout (set when project setup is saved).
